Self-Management Support Worker

Role

LCIL is a user-led organisation providing a range of services that enable Disabled People to live independently in the community of their choice.

LCiL are recruiting for a part time Self-Management Support Worker to support the effective development and delivery of the Supported Self-Management Programme, working in partnership with Edinburgh Health and Social Care Partnership’s Long Term Conditions Programme across the four localities in Edinburgh. The aim of this programme is to ensure that people living with long term conditions in Edinburgh receive the right help at the right time and are empowered to be regularly involved in, and responsible for, their own health and wellbeing.

LCIL is looking for someone with:

  • Experience of developing and shaping a programme with voluntary organisations and/or statutory bodies
  • Experience of the design and delivery of information services whilst working with individuals on a time limited, 1-1 basis
  • Experience of developing strategic relationships across different sectors
  • Understanding of Independent Living philosophy and Social Model of Disability
