We have an exciting new opportunity to deliver Community Links Workers in Deep End GP practices in the North East and North West of Glasgow.

The Community Links Workers will be based in GP practices and support people to live well in their own communities and provide specialist, person centred services to people who have health, social care or wellbeing needs. The aim of the Links Workers is to support people to identify barriers to their overall wellbeing and link them with the appropriate tools, community support or agency to overcome these. A key part of the role will be developing connections between patients, practice staff and local community providers.

If you have a passion for supporting individuals to make sustainable change, have a sound knowledge of social, economic and other factors which may impact on the health and wellbeing of individuals and communities, and are qualified to SVQ Level 3 in Social Services and Health Care or Community Development we would love to hear from you.
