Role

Getting Better Together Ltd is a Community Led Health Improvement organisation based in Shotts North Lanarkshire.

Location: Employment will mainly be based in the hospital acute sites, community hospital and rehabilitation units across Lanarkshire.

For this post we are seeking a highly motivated individual who would engage with patient, families, and carers to offer advice on possible community supports and refer to appropriate organisations when required. The service aim is to encourage clients to engage with additional supports with the anticipated outcomes of reducing loneliness and isolation, improving self-management of conditions, encouraging independence and physical activity. The service offers support throughout the patient’s journey, and this can continue post discharge.

In this role you will see people facing all kinds of health and wellbeing challenges. Your remit will be to support clients through their hospital journey and linking them with the right tools, community support or services to improve their wellbeing. This is done using face to face personal outcome conversations and telephone work.
