Role
Due to the current staffing configuration of our Carntyne service, we require a Female Recovery Worker to meet the support needs within the team.
Recovery Workers work in small collaborative teams to provide people with recovery focused support to enhance their mental wellbeing. In line with Penumbra’s values, Recovery Workers assist people who use the service to work towards their individual outcomes and identified goals, as detailed in their personal plan.
Special Conditions: Local travel
Hours include: Evenings, Weekends, Sleepovers (Supplement paid per sleepover worked). Female worker required

Recovery Practitioner (Peer) - Peers 4 HOPE

To assist people who use the service to establish and maintain a meaningful and fulfilling life in the community. Playing a central role in the planning and provision of quality, recovery focused support.
Responsibilities
- Establish an effective relationship with people who use the service in accordance with Penumbra’s policies and procedures.
- Develop outcome-based personal recovery plans for each service user, which promotes self –management and social inclusion.
- Co-ordinate delivery of recovery focused support.
- Deliver community-based well-being workshops that promote positive mental health.
- Provide practical support and supervision to Recovery Workers and/or Peer Workers as required.
- Develop effective relationships with other groups and agencies in your area and take opportunities to promote mental health awareness in the wider community.
- Be responsible for maintaining the relevant systems of documentation.
- Provide accurate information and reports as required.
- Provide formal input to Care Management reviews as required.
- Be flexible and responsive to meet the changing needs of the service users and the organisation, within the agreed criteria.
- Work effectively with other members of the team, and liaise effectively with families, agencies, colleagues and other stakeholders.
- Participate in training activities and meetings as required.
- Uphold Penumbra’s Code of Practice.

During the Covid-Pandemic this post is working from home. Staff will return to community support based on government guidance.
About the role
You will be based in Harbours GP , providing a specialist, person centred service to people who have health, social care or wellbeing needs within that geographical area.
The aim of the Link Worker is to support people to identify barriers to their overall wellbeing and link them with the appropriate tools, community support or agency to overcome these. This will be delivered using a Social Prescribing model.
The Link Worker role is three fold - patient facing, practice facing and community facing - developing connections between patients, practice staff and local community providers is crucial to the programme.
What we are looking for
If you have a passion for supporting individuals to make sustainable change, have a sound knowledge of the East Lothian area and the social, economic and other factors which may impact on the health and wellbeing of individuals and communities, are qualified to SVQ Level 3 in Social Services and Health Care or Community Development or equivalent then we would love to hear from you.

Assertive Outreach is a well-tested model supporting an agile and flexible method to prevention, intervention and engaging with those most vulnerable. The new model for our Assertive Outreach Plus Service within Transform Forth Valley is being developed as we move away from our current funding and respond effectively to known local need.
The Assertive Outreach Plus Service will:
- Support individuals and families who experience challenge; either financial, societal or in relation to their health and wellbeing
- Tackle inequality, where life chances are diminished because of deprivation.
- Employ a whole person, systematic and partnership approach to achieving change.
- Deliver support acknowledging that issues are interrelated and require to be addressed collectively for individual outcomes to be improved.
The Assertive Outreach Plus Service will respond to several co-existing challenges that individuals face to provide a seamless whole person response to care and support.
The Project Worker will function as an integral part of this team, working in a flexible and agile way within communities providing individualised support on a range of issues. A key aspect will be coordinating actions with a wide range of partners including Police Scotland, Social Work, and other Third Sector organisations. The Project Worker will develop care plans with the client to ensure their aspirations are met.
