Role

Background

Community Link Workers (CLW) are based within GP practices and provide a person-centred service that is responsive to the needs and interests of people living within an area affected by socio-economic deprivation. CLWs support people to:

  • identify issues that affect their ability to live well
  • identify personal outcomes and priorities that they would like to address
  • overcome any barriers to addressing these
  • link them to local and national support services and activities.

Based within the local Third Sector Interface (TSI), the CLWs also build relationships and processes between the GP practice and community resources, statutory organisations, other health services and voluntary organisations.

Role

The Lead CLW Role has 2 elements: to provide leadership and day to day management and supervision support to a team of CLWs and to manage a caseload of clients within a dedicated GP practice(s) in Inverclyde. Additionally, the Lead CLW will utilise available data to raise awareness about the work the CLW team does and the impact it has on people across Inverclyde. This will require strong working relationships with partners from both the Third and public sectors and with communities throughout Inverclyde.
