Calman Trust works mainly with young people aged 15-25 who experience difficulty in the transition to independent living or the world of work, and are not yet ready or able to benefit from mainstream learning or training programmes. We offer the young person a bespoke combination of learning and support, believing that, with support to overcome the challenges they experience on the way to independent adulthood, young people are capable of achieving great things.
We are recruiting for the key position of Case Manager to work within the team, ensuring that the young person receives the support he/she needs to sustain participation and progress.
The Case Manager is accountable to the Head of Learning and Development (HOLD), with responsibility for ensuring that young people coming through the service have the support they need to manage any personal barriers and sustain progression through their learning, to achieve their destination.
The Case Manager will ensure that every young person has a holistic plan of support and learning which reflects their aspirations, an understanding of their barriers and support needs and a plan to address these, whilst pursuing their learning priorities and access to suitable opportunities.
