We’re recruiting for an exciting new role here at SNAP - a ‘Transitions Service Lead’.
If you are someone who understands the value of Youth Work - having fun, yet tailored support and have experience working with children and young people with additional support needs, this job is for you!
This role will be required to lead in the planning and delivery of our newly created and already well received SNAP Transitions Service, along with supporting some of our After School Clubs and one of our weekly Youth Clubs.
Our Transitions Service supports young people with additional and/or complex needs through the transition of leaving secondary education. Through this vital service, we offer a range of activities that help these young people achieve personalised outcomes and gain important life skills which will help prepare their transition into adult life.
This role will be a minimum of 25 hours, but will likely be 30+ hours a week, spread across Mondays – Fridays. With this being such a new service, it is a great opportunity for the right person to be creative and develop partnerships with other organisations to collectively work for the best outcomes for our young people. As the Transitions lead, good organisation, management and people skills are crucial.
Driving licence preferred.
Applicants should ideally possess an SVQ3 in Health & Social Care or similar such as a PDA in Youth Work.
