Chest Heart & Stroke Scotland (CHSS) is an independent Scottish charity, whose mission is to be the community of support, kindness and challenge where people can help each other, can secure the expert help they need, and collectively advocate for the care that matters to them. We offer vital advice, support and information to those affected so that they can live the lives they want to live and influence public policy to ensure that people get the services that they need. The main purpose of the post of the Community Support Coordinator is to carry a caseload for community support services in your designated area.

Our Community Support Services are designed to support people to do what matters to them and progress towards self-management so they can live life to the full and as independently as possible. We provide volunteer and staff led, one to one and group services in the community and hospital settings.

We are looking for someone with a positive attitude towards people with disabilities and long-term conditions; an understanding of the challenges faced by people with cardiac, respiratory and stroke conditions as well as Long Covid; an understanding of community service provision; the ability to recruit, manage and motivate a team of volunteers; and excellent communication and interpersonal skills.

Applicants must have a car and a current valid driving licence (expenses are paid at mileage rate).
