WDP Harrow is an established community Integrated service that is well performing and highly functioning. The service consists of both clinical and psychosocial and is CQC registered to provide a range of clinical interventions including BBV & health interventions, OST, and community detox, as well as robust family and carers and service user involvement provision.
An exciting opportunity has arisen for a Safeguarding Families/Safeguarding Adults Recovery Practitioner to join WDP Harrow.
The Safeguarding Families/Safeguarding Adults Recovery Practitioner will be responsible in proving support to the Adult Social Care team to identify service users with problematic drug and/or alcohol and increase referrals from Adult Social Care to the Substance Misuse Service. In addition, they will undertake joint home visits to engage vulnerable adults, specifically those on the threshold or those engaged in the Criminal Justice System, including those with a Learning Disability and Mental Health need. They will support the current Dual Diagnosis work in the Mental Health acute, community sites and support the development of Mental Health pathways from prison custody to community.
The successful candidate will be highly motivated, committed, recovery focused and use their drive and innovation to ensure a consistent high-quality service, showcasing good practice, high performance and excellence in given areas of expertise.
The successful candidate will predominantly be working within WDP Harrow.
