Improving the Cancer Journey (ICJ) is being introduced in South Lanarkshire, funded by Macmillan Cancer Support as part of the Lanarkshire wide Improving the Cancer Journey Programme which supports people’s non-clinical needs following a cancer diagnosis and is a partnership between NHSL, South Lanarkshire Health & Social Care Partnership, VASLan and Macmillan.

We are looking for four colleagues to join us in partnership with Macmillan Cancer Support and the Health & Social Care Partnership to deliver this essential service.

The ICJ wellbeing practitioner is a key role within the service that will work with people affected by cancer, their loved ones, and carers to ensure their non-clinical support needs are met, engaging with the person diagnosed with cancer to discuss their support needs, and connecting them with community based supports that can meet those needs.

To support capacity building within the third sector in Dundee to access funding from funders, leveraging money into the city.

This includes:

• Lobbying in relation to future funding; leading on external funding bids for third sector organisations; co-ordinating the development of project business plans as required; supporting and facilitating partnership bids within the Third Sector.

• Establishing strategic funding relationships; maximising funding opportunities available to Third Sector organisations to support Dundee City Plan priorities and Dundee Health and Social Care Partnership outcomes and to better the lives of people and communities across Dundee.

• Capacity Building to support third sector organisations to become self-sustaining, such as bid writing, project development, monitoring & evaluation, fund management

We are recruiting a Family Group Decision Making Coordinator to join our team in South Lanarkshire. This is an exciting opportunity to be part of our partnership with South Lanarkshire local authority, to extend and develop a rights-based approach to working with families and communities through Family Group Decision Making, using an Early help approach.

Although this is a small service, you will be working alongside wider multiagency partners. You will have the opportunity to support a new way of working within South Lanarkshire.

The post holder will engage with children and families to ensure their voices are at the heart of planning and decision making in matters that affect them. We are therefore looking to recruit a dynamic, experienced individual who can drive the successful development and growth of this innovative partnership.

This is a fixed term, full time role of 35 hours per week, funded initially until 31 March 2024. Flexible working, job share, and part time will be considered.

Voluntary Action North Lanarkshire and Community Solutions Programme which is the community capacity building and carer support arm of Health and Social Care North Lanarkshire have secured funding through the Scottish Government Support in the Right Direction fund to develop a project supporting people to access social and community care as well as local community supports and services in North Lanarkshire. The project is hosted and managed by North Lanarkshire Disability Forum (NLDF).

The project compliments and adds to existing local social work provision by focussing one to one support for people who are not eligible for a social care budget to ensure they are better supported within the community and are able to access and manage support that maximises their assets and helps meet personal outcomes. This post will run until March 2024 with a possible extension (funding dependent) extending the reach of the project across all localities within North Lanarkshire.

We are looking for a strong communicator to strengthen our existing team in helping people through this difficult time by further developing our online groups and connecting people to other local resources, this involves having conversations aroundpractical and emotional issues about community supports, a place to discuss the current climate and the issues for people with or without an SDS budget around a particular issue or topic, e.g. mental health, housing, employment, gardening, art.

This role is to provide an opportunity for people to discuss issues in an informal way. The ideal person will also have experience of marketing and promoting projects to engage and encourage involvement of the target group.This post is subject to membership of the PVG scheme.
