You will be part of Scotland’s leading charity providing support to people with chest, heart and stroke conditions to live life to the full again. Our Hospital to Home services form a nationwide network of local support groups, nurses, volunteers and one-to-one support teams helping families adjust to life with a heart or lung condition or after a stroke.

This is an exciting new role; we are looking for a stroke nurse who has experience of working with patients who have had a stroke or TIA. This is a unique post offering the opportunity to provide direct care to patients following discharge from hospital as well as providing the scope to deliver stroke education to staff aligned to the National Stoke Education Pathway. As Stroke Nurse/Stroke Educator, you will be key in providing support to stroke survivors, their families, and carers to ensure they receive the help and support they require to live the best lives they can. You will also be key in providing stroke education to staff and colleagues within the NHS, Health & Social Care Partnership, and other partner organisations working with colleagues in the CHSS Education Team promoting the highest standards of care. This is a permanent full-time post of 32.5 hours per week. Driving is a requirement of this post and a clean driving licence and access to a vehicle is essential.
