We are looking for a Transitions Worker to join our Family Support Team to work with 16-21 year olds who have palliative care needs and use our hospice services.

The role

  • Carry out assessments to support the holistic needs of young people
  • Enable young people and their families to have their voices heard and choices respected as they transition from children’s to adult services.
  • Support will be offered within the family’s home, hospice and community settings.
  • Manage a caseload
  • Coordinate, develop and deliver a service provision designed to help a young person’s transition journey with recognition of the impact this may have upon their wider family.

You will have

  • Experience of working with young people with complex, palliative care needs
  • Excellent knowledge of legislation relating to transition, informing community care packages and self directed support.
  • Strong communication skills
  • Group facilitation skills
  • Can manage difficult conversations and sensitive information
  • Work autonomously AND be a team player
  • Can deal with competing demands
  • Ideally you will be qualified to HND level in a relevant subject area

Staff and volunteers work together as one team, focused on the common goal of reaching every family in Scotland who needs our support. All CHAS staff will at some point work with or alongside volunteers and will be expected to support volunteers.
