Oxgangs Care: We aim to support people over 60 in Edinburgh EH10 & EH13 who are disadvantaged by illness, disability or at risk of social isolation.
The Community link worker posts are funded by Merck, Sharp and Dohme (MSD) and Edinburgh Health & Social Care Partnership and will be delivered in partnership with GP Practices and the 3rd sector to support people to live well through strengthening connections between community resources and primary care.
We are looking to recruit to the two posts of Community Link Worker who will work with socially isolated older people in local GP practices as part of the practice team. Link workers will provide 1:1 person-centred support to people in contact with a GP practice team. This postholder will work with individuals to enable them to identify personal outcomes and priorities for their health and well-being and link them to local and national support services and activities. You must have a strong understanding of the reality of social isolation and loneliness. Experience of working with people in a community setting and extensive effective interpersonal skills are essential
