Role

The Community link worker post is funded by the Scottish Government and delivered in partnership with GP Practices and the third sector to support people to live well through strengthening connections between community resources and primary care.

We are looking to recruit to the post of Community Link Worker who will work with socially isolated older people in our local GP practice as part of the practice team, but be based at LifeCare. The role of the Community Link worker will be to provide 1:1 person-centred support to people in contact with thug practice team. The postholder will work with individuals to enable them to identify personal outcomes and priorities for their health and well-being and link them to local and national support services and activities.

You must have a strong understanding of social isolation and loneliness.

Experience of working with people in a community setting and extensive effective interpersonal skills are essential.
