You will lead the staff team in using innovative and creative approaches to support the people we support to live active and fulfilling lives within their local community to achieve their personal outcomes.
Ideally you will have excellent communication, organisational skills and an attention to detail with experience of working collaboratively with the people we support, their families and in partnership with local authority health and social work teams. You will lead and support staff in the writing of support plans ensuring the health and wellbeing of the people we support is met.
