Job Purpose
As our Substance Misuse Nurse, you will provide a range of Health and Wellbeing, and harm reduction interventions to service users in accordance with the agreed model of service delivery. You will deliver high standards of nursing care to substance misusers and be responsible for the assessment of care needs, development, implementation and evaluation of programmes of care.
Main Responsibilities
You will be an advocate for our patients, working closely with them to identify the physical, psychological, spiritual and social support they need. The postholder will work flexibly providing a high standard of nursing care in a variety of settings, promoting the health and well-being of service users whilst maintaining a safe, caring and therapeutic environment. As a Substance Misuse Nurse you will participate as an active member of the nursing and multi-disciplinary teams, accountable for your practice, and work in partnership with other health care professionals, psycho-social case managers, patients and others in the delivery of care.
You will support the delivery of specific healthcare clinics or groups, designed to support and educate clients, providing them with practical tools to overcome life’s challenges, this will include pre-session planning and post-session evaluation.

You will manage our Renfrewshire Community Link Worker service ensuring that a specialist, person centred service is delivered by a team of link workers to people who have health, social care or wellbeing needs. The link workers are based within GP practices in Renfrewshire and you will be expected to manage all aspects of the programme delivery locally; including relationships with the Health and Social Care Partnership, GP and Practice Staff and wider community stakeholders. The Link Worker programme is three fold - patient facing, practice facing and community facing - supporting, identifying and developing connections between patients, practice staff and local community providers is crucial to the programme, and to the role of Community Link Work Manager.

Our service in Argyll & Bute is commissioned to provide a recovery service for people concerned about their own or someone else’s drug or alcohol use and delivers psycho social interventions in both one to one and group settings for adults entering or sustaining their recovery from drug/alcohol use.

We meet people at a range of locations across East Ayrshire and at our base in Kilmarnock. You will be part of a skilled and motivated team supporting people to make positive changes.

We are looking for an experienced Community Navigator to join our team in the Scottish Borders. This role is designed to assertively engage individuals in the Community setting and provide support to access the residential rehabilitation pathway. You will need experience of working in substance misuse, offering one-to-one and group work facilitation, first class assessment and support planning skills, risk management and safeguarding.
A good working knowledge of local services and appropriate referral pathways, and support agencies. Ideally the successful candidates will have outstanding communication skills and the ability to work effectively as a team player in a collaborative and supportive manner. You will be someone who thrives under pressure, has good time management and prioritisation skills with the ability to work well with others.
In addition you will need to be positive, confident and well organised with experience of working with people in a recovery, health, social care or criminal justice setting.
The successful candidate will be required to deliver services within the community so a driving license is essential.
